Description ADAMTS-13 is a plasma enzyme that is responsible for cleaving of unusually large vonWillebrand multimers into smaller normal fragments. Absent or low ADAMTS-13 activity allows formation of platetlets micthrombi in small vessels due to presistence of ultra large vonWillebrand multimers, generating the clinical sequelae of TTP. ADAMTS-13 activity is measured by Fluorescence Rensonance Energy Transfer(FRET) with a synthetic substrate. ADAMTS-13 inhibitor is determined by using mixing studies. The ADAMTS-13 antibody assay detects IgG antibody seroloically by ELISA. ADAMTS-13 a reflexive testing algorithm. Activity is always performed. If activity result is ≤ 30%, the inhibitor assay will be perfomred. If inhibitor result is ≤ 0.7 inibitor units, the antibdoy assay will be performed.

Collection Instructions Completely fill the Blue Top (sodium citrate) tube. Underfilled tubes will not be accepted. Mix sample immediately by gentle inversion at least six times.
Specimen Handling Outpatient specimens: Specimens should be centrifuged and frozen within 4 hours of collection to generate platelet-poor plasma. A minimum of 0.5 ml frozen citrated plasma is needed per test ordered. Inpatient specimens: Transport at room temperature. Specimens should be received in lab within 2 hours of collection.
Rejection Criteria Hemolysis, clotted, short sample, >4 hrs old (if not frozen), EDTA Tube.
